April 2024 — Monthly Briefing¶
1. What was read¶
701 research articles from 32 of the 33 journals on the roster.
A further 156 records (18% of the 857 this month's harvest returned) carry no abstract and are not research: news, correspondence, editor highlights and obituaries. PubMed types them Journal Article and nothing else, so the publication-type filter cannot see them — across the whole index they are 22.7% of it and 35% of general, and 3,801 of them are JAMA. Every count below is over the full harvest and is inflated by this; the figure above is not.
| General | Cancer | Immune | Bioinfo | Sysbio |
|---|---|---|---|---|
| 383 | 181 | 66 | 203 | 24 |
Volume swing against the median month: bioinfo -28%
Check the source before reading anything into this. Two benign causes account for most of these. bioinfo every January carries the Nucleic Acids Research database issue and runs high. And several journals simply deposit nothing in some months: across the 44 months read, 4 of 33 journals are absent from at least one, worst being Nat Mach Intell (20 months missing, 81 papers in total) and Bioinformatics (4 months missing, 2,905 papers in total). This month is missing Bioinformatics. Queried directly, PubMed returns zero for those journal-months too — the holes are in the source, reproduced exactly by re-running the harvest. So a swing here is not evidence that a field went quiet, and is not usually evidence that the pipeline broke either.
Largest contributors: Nature 98, Nucleic Acids Res 94, Science 80, JAMA 55, J Immunother Cancer 43, Lancet 38.
2. Net change on the thread curve¶
| Thread | This month | Cumulative | Status |
|---|---|---|---|
agentic-ai |
— | +4 / −0 | too-early |
cell-therapy |
+3 / −2 | +99 / −32 | forming |
ctdna-mrd |
−2 | +32 / −15 | forming |
foundation-models |
+3 / −2 | +41 / −29 | splitting |
immunotherapy-resistance |
+4 / −1 | +105 / −36 | forming |
interception |
+4 | +66 / −9 | forming |
method-credibility |
−13 | +3 / −189 | crisis |
neoantigen-vaccines |
+3 / −2 | +45 / −19 | forming |
neuro-immune-microbiome-axis |
+8 | +76 / −8 | forming |
next-gen-checkpoints |
+2 / −1 | +31 / −22 | splitting |
single-cell-3d-genome |
+3 / −2 | +29 / −9 | forming |
suppressive-population-inversion |
+2 / −1 | +25 / −32 | contested |
Status is derived by rule, not asserted. Fewer than 5 cards → too-early; then refutes ≥ 2×supports → crisis; refutes > supports → contested; supports ≥ 2×refutes → forming; else splitting.
3. The evidence that moved it¶
A pan-cancer map of the metastatic tumour microbiome. 38599211 (Cell)
integrates metagenomics, genomics, transcriptomics and clinical data over 4,160
metastatic biopsies from 26 cancer types, and reports organ-specific microbial
tropism, anaerobes enriched in hypoxic tumours, an association between microbial
diversity and tumour-infiltrating neutrophils, and Fusobacterium associated
with resistance to checkpoint blockade in lung cancer. Longitudinal sampling adds
which bacteria are depleted under treatment. It is a resource on a scale the
neuro-immune-microbiome-axis thread has nothing else like, and it is one study.
Corrected 2026-08-27
This paragraph was headed "Two independent pan-cancer metastasis microbiome
maps, same journal, same month" and argued that "convergence at this grade is
what separates the neuro-immune-microbiome-axis thread from
next-gen-checkpoints, where nominations appear once and are never
repeated". There was no second map. 38670070 is a three-sentence
Cell preview of 38599211, in the same issue, reporting no data of
its own; its title is nearly a copy of the title it previews, which is how it
was carded as a second resource. One study was counted twice and the doubling
was then used as the argument. The preview's card is now neutral.
Alongside it 38662827 (Science) reports that vitamin D regulates microbiome-dependent cancer immunity, linking diet, microbiota and antitumour immunity in one causal chain, and 38479384 (Immunity) shows microbiota-modified bile acids promoting colorectal cancer growth by suppressing CD8 T-cell function.
A next-generation checkpoint in a registrational-track setting. 38504015 (Nat Med): neoadjuvant nivolumab or nivolumab plus the LAG-3 inhibitor relatlimab in resectable oesophageal cancer, phase Ib with ctDNA. This matters as a baseline. By late 2025 this corpus reports that across all of 2025 not one newly nominated checkpoint entered a phase 1, and that October 2025 returned zero hits for the entire target list. April 2024 shows what the category looked like when it was still functioning.
4. Refutations¶
(a) Landing on a tracked thread¶
The shared-neoantigen design is tested and it does not work. 38538867 (Nat Med) reports phase 1 interim results for a 20-epitope shared-neoantigen vaccine — a chimp adenovirus prime and self-amplifying mRNA boost — given with ipilimumab and nivolumab to patients whose tumours carried one of the encoded HLA-matched mutations, 18 of 19 of them in KRAS. The objective response rate was 0%, median progression-free survival 1.9 months, and two patients had dose-limiting toxicities. The immunology is the sharper problem: T cell responses went preferentially to the HLA-matched TP53 epitopes in the vaccine rather than to the KRAS neoantigens the patients' tumours actually expressed. That is an immunodominance hierarchy inside the construct, and it is an argument against multiepitope shared-neoantigen vaccines as a class rather than against this one product. The trial's own response was to build a KRAS-only version.
Corrected 2026-08-27
This paper was in section 3 under "The vaccine thread routes around its
manufacturing objection", with the reading that "shared rather than
personalised is the design that survives if per-patient manufacture does
not". That was written from a title that names the design and not the
outcome. The outcome is above; the card is now refutes on
neoantigen-vaccines, which is the only supports-to-refutes reversal on a
trial in this corpus.
Corrected 2026-08-27
This section previously carried 38490738 (Genome Res) —
"experimentally evolved Drosophila populations may fail to establish an
effective piRNA-based defence. A textbook mechanism does not reproduce under
direct experimental test." The paper audits nothing. P-elements were
introduced into naive Drosophila erecta lines and followed for more than 50
generations in three replicates; piRNA defence established in two and failed
in the third, where the ping-pong cycle never activated despite
piRNA-cluster insertions and siRNAs being present. The replicate that failed
is the result — activation is a stochastic event in a population — not a
failure to reproduce anything published. "May fail" in a title was read for a
general moral it does not carry. The card is neutral and carries no thread.
(b) Method credibility¶
38637899 (Genome Biol): library size confounds biology in spatial
transcriptomics data. A technical covariate is being read as biological signal
across an entire modality, and the modality is the one single-cell-3d-genome
claims downstream pipelines must ingest. Every atlas built without this
correction inherits the error — the same shape as the November 2025 finding that
standard batch integration destroys real biological signal.
38413730 (Nat Med) is the counterweight and belongs in section 3 as much as here: adapted large language models outperform medical experts in clinical text summarisation. The clinical-AI evidence in 2024 runs in both directions from the start.
