Program notes — ASCO GU 2026¶
Internal working file — pre-meeting prep style (the style of asco-2026/program-notes.md), reconstructed post-meeting. Tracks, day-by-day shape, expected LBAs, discussant watch list, key dates. Status: post-meeting, 10 weeks out (today: May 11, 2026). Pre-embargo language preserved for fidelity to the working-notes style; speculative entries that were resolved on-the-day are flagged with the resolved value in italic.

Venue + format¶
- Venue: Moscone West, San Francisco, CA (the standard ASCO GU venue; co-located overflow at the Marriott Marquis for press / sponsor booths in prior years, varies year-to-year).
- Attendance: ~3,500 total (in-person + virtual hybrid). Roughly 2,400 in-person + 1,100 virtual per ASCO registration patterns from prior years.
- Format: 3 days, Thu–Sat. Hybrid (in-person + virtual). Co-sponsored by ASCO, ASTRO (American Society for Radiation Oncology), and SUO (Society of Urologic Oncology).
- Daily theme structure: ASCO GU has historically organized days by disease group — Day 1 RCC, Day 2 urothelial/bladder, Day 3 prostate (the highest-volume group). 2026 followed the same pattern. ~3 oral abstract sessions + ~10 rapid-orals + multi-disciplinary panel discussions per day. ~1,200 abstracts total.
- Plenary structure: Unlike ASCO main, ASCO GU does not have a dedicated single "Plenary Session" slot — instead, the LBAs are distributed across the disease-group days. LBA418 / LBA417 / LBA630 / PEACE-3 OS are the four practice-changing slots.
Day 1 — Thursday Feb 26 — Renal Cell Carcinoma¶
The RCC day was anchored by Merck's belzutifan franchise.
| Slot | Trial | Drug / regimen | Indication | Status |
|---|---|---|---|---|
| LBA418 | LITESPARK-022 | Pembrolizumab + belzutifan vs pembro mono | Adjuvant high-risk ccRCC post-nephrectomy | DFS HR 0.72 (95% CI 0.59–0.87, p=0.0003) — practice-changing; sNDAs under FDA priority review at time of GU readout |
| LBA417 | LITESPARK-011 | Belzutifan + lenvatinib vs cabozantinib | 2L+ mRCC (post-IO/TKI) | Mixed PFS/OS readout per Motzer presentation; details to reconcile against abstract |
| Oral | CYTOSHRINK | SBRT + nivo/ipi vs nivo/ipi | Metastatic RCC | Exploratory neoantigen-release hypothesis |
| Posters / rapid | ccRCC neoadjuvant cabo/nivo; non-clear-cell RCC subtypes | — | — | Signal-generating |
Day 2 — Friday Feb 27 — Urothelial / Bladder¶
The bladder day was anchored by EV+pembro perioperative MIBC. This was the headline of ASCO GU 2026 by news cycle.
| Slot | Trial | Drug / regimen | Indication | Status |
|---|---|---|---|---|
| LBA630 | KEYNOTE-B15 / EV-304 | Enfortumab vedotin + pembrolizumab vs gem/cis | Perioperative MIBC, cisplatin-eligible | EFS HR 0.53 (95% CI 0.41–0.70, p<0.001); 24-mo OS 86.9% vs 81.3% — displaces cisplatin in curative-intent MIBC |
| Oral | IMvigor011 | ctDNA-guided atezolizumab vs observation | Adjuvant MIBC, ctDNA+ | Positive — establishes ctDNA-guided adjuvant as deployable |
| Oral | NIAGARA biomarker analyses | Durvalumab perioperative + biomarker | Perioperative MIBC | ctDNA dynamics sub-analysis |
| Oral | SunRISE-2 | Gemcitabine intravesical system + cetrelimab | MIBC alternative to CRT | Missed primary BI-EFS; signal-only CR rates |
| Rapid orals | FGFR3 mUC (erdafitinib follow-ups, novel FGFR3 selective inhibitors); ADC sequencing post-EV | — | — | Class read-through |
Day 3 — Saturday Feb 28 — Prostate¶
The prostate day was the deepest — four-plus oral sessions across mHSPC, mCRPC, biomarker-selected populations, and radioligand therapy.
| Slot | Trial | Drug / regimen | Indication | Status |
|---|---|---|---|---|
| Oral | PEACE-3 / EORTC 1333 | Enzalutamide + Ra-223 vs enza | Bone-mets mCRPC 1L | OS HR 0.76 (95% CI 0.60–0.96, one-sided p=0.0096) — re-validates alpha-emitter combo with modern bone protection. Follow-up to ESMO 2024 rPFS readout. |
| Oral | PSMAaddition | ¹⁷⁷Lu-PSMA-617 + ADT + ARPI vs ADT + ARPI | 1L mHSPC | Positive rPFS per topline — moves RLT from 3L+ (VISION) to 1L mHSPC. OS mature follow-up at ASCO main / ESMO 2026. |
| Oral | MEVPRO-3 | Mevrometostat (EZH2i) + enzalutamide | mCRPC | Ph3 interim; Ph1 had shown rPFS 14.3 vs 6.2 mo |
| Oral | BRCAAway | Olaparib + abiraterone (upfront combo vs sequential) | HRR-altered mCRPC | Tests upfront combo vs sequential PARPi/ARPI |
| Oral | CAPItello-281 | Capivasertib + abi vs placebo + abi | PTEN-deficient mHSPC | Positive per topline; PRO assessments emphasized — first PTEN-biomarker-selected Ph3 in mHSPC |
| Oral | ²²⁵Ac-PSMA first-in-human | Actinium-225 PSMA dose-escalation | mCRPC (post-¹⁷⁷Lu-PSMA salvage) | Fred Saad presenting; signal-generating alpha-emitter |
Discussant watch list¶
ASCO GU discussant identity is signal-rich; the disease-group seniors typically frame the post-meeting narrative.
- Prostate (PEACE-3 OS, PSMAaddition, CAPItello-281): likely Karim Fizazi (Gustave Roussy), Stephen Freedland (Cedars-Sinai), Tanya Dorff (City of Hope), Fred Saad (Université de Montréal — also a presenter for ²²⁵Ac-PSMA). The mHSPC vs mCRPC line-of-therapy framing is the recurring question.
- RCC (LITESPARK-022 / LITESPARK-011): likely Robert J. Motzer (MSK — also a presenter), Toni Choueiri (Dana-Farber), David McDermott (Beth Israel Deaconess). HIF-2α + IO sequencing is the framing.
- Bladder (KEYNOTE-B15 / EV-304): likely Matthew Galsky (Mount Sinai), Thomas Powles (Barts), Andrea Apolo (NCI). The "cisplatin displacement" framing dominated the post-meeting cycle.
- Rare GU (penile / testicular / adrenal): smaller disease-group seniors — Curtis Pettaway (MDACC, penile), Lance Pagliaro (Mayo Clinic, testicular).
Forward-looking flags¶
ASCO GU readouts typically chain into one of:
- ASCO 2026 main meeting (May 29 – Jun 2):
- PROTEUS LBA1 (apalutamide perioperative HRLPC) — the ASCO main Plenary GU sibling to ASCO GU's prostate slate. PROTEUS covers the localized / perioperative space; ASCO GU 2026 covered mHSPC + mCRPC.
- KEYNOTE-B15 / EV-304 OS-mature update expected.
- PEACE-3 bone-mets subgroup follow-up.
-
LITESPARK-022 OS interim.
-
ESMO 2026 (Oct 23 – 27): typical venue for OS-mature follow-up of GU trials whose primary DFS / PFS endpoint hit at GU. PSMAaddition OS mature; CAPItello-281 OS mature.
-
SUO 2026 (Dec): surgical-oncology perspective on perioperative MIBC and prostate trials. Cystectomy outcomes follow-up for KEYNOTE-B15.
-
AUA 2027 (May): urology perspective; NCCN guideline reconciliation likely complete by then.
What's speculative vs. confirmed (post-meeting)¶
Confirmed (from public coverage; to be reconciled against ASCO Meeting Library abstract): - Meeting dates, venue, format (asco.org/gu; medically.gene.com) - LITESPARK-022 DFS HR 0.72 (Merck press release, post-meeting trade-press confirmation) - KEYNOTE-B15 / EV-304 EFS HR 0.53; 24-mo OS 86.9% vs 81.3% (Merck + Pfizer/Astellas press releases) - PEACE-3 OS HR 0.76 (EORTC press release; OncLive recap) - LBA abstract numbers (LBA418, LBA417, LBA630)
Speculative — pending Meeting Library reconciliation: - LITESPARK-011 exact PFS / OS numbers (trade press reports "mixed"; abstract permalink needed) - PSMAaddition full headline numbers (positive topline; OS interim only) - MEVPRO-3 Ph3 interim numbers (only Ph1 numbers in public coverage so far) - BRCAAway PFS magnitude - CAPItello-281 PRO endpoints - ²²⁵Ac-PSMA dose-escalation safety / activity details - Named discussants (above watch list — to confirm against the official program archive) - Rare-GU rapid-orals (InPACT, testicular, adrenal)
Dead ends (for now)¶
- Full discussant transcripts / slide decks: ASCO GU does not pre-release slides; only available via ASCO Virtual (paid registration) or post-presentation press coverage.
- OS-mature data for ASCO GU 2026 positive trials: by definition, not yet available. Forward-looking flags above.
- NCCN guideline reconciliation: typically 60–90 days post-meeting; some updates already in place by May 11, 2026 (KEYNOTE-B15 / EV-304 perioperative MIBC anticipated in NCCN bladder update; LITESPARK-022 anticipated in NCCN kidney update following sNDA action).
