SARC041 — Abemaciclib vs placebo, advanced dedifferentiated liposarcoma¶
Abstract: LBA2 · Session: Plenary, Sun May 31, 2026, Hall B1 · Presenter: Mark A. Dickson, MD (Memorial Sloan Kettering) · Embargo: lifted 7:00 AM CT May 31
At a glance¶
- Sponsor: Investigator-initiated (SARC cooperative); funded by Eli Lilly
- PI / presenter: Mark A. Dickson (MSK)
- NCT ID: NCT04967521
- Phase: 3
- Design: Randomized, double-blind; abemaciclib vs placebo
- N: 108 (recurrent or metastatic dedifferentiated liposarcoma)
- Primary endpoint(s): PFS — met
- Comparator: placebo
- Indication: advanced dedifferentiated liposarcoma (DDLPS), not amenable to surgical resection
- Line: advanced/metastatic
Headline result¶
Median PFS 9.7 vs 1.5 months; ~62% reduction in risk of progression or death (HR ≈ 0.38). This is the first-ever positive phase 3 in dedifferentiated liposarcoma — a subtype with poor prognosis, high relapse, and no prior systemic standard.
Mechanism / class¶
Abemaciclib is a CDK4/6 inhibitor. DDLPS is characterized by CDK4 amplification (12q13–15 amplicon), providing a direct biologic rationale for CDK4/6 blockade.
Discussant takeaway¶
Framed as establishing a new systemic option / potential standard of care in a setting with essentially no effective drugs; the very short placebo-arm PFS (1.5 mo) underscored the unmet need.
Practice-changing?¶
Likely yes for the DDLPS subset — first randomized evidence of benefit; positions abemaciclib as a new standard for unresectable/advanced disease.
