Headline result¶
EPCORE DLBCL-1 is the first phase 3 study of a CD3xCD20 T-cell–engaging bispecific monotherapy to improve PFS over chemoimmunotherapy in R/R DLBCL. Epcoritamab improved PFS versus investigator's choice chemotherapy (HR 0.74; 95% CI, 0.60–0.92 — per Genmab/AbbVie topline, Jan 16, 2026). Improvements were also reported in complete response rate, duration of response, and time to next treatment. Overall survival favored epcoritamab numerically but was not statistically significant (HR 0.96; 95% CI, 0.77–1.20 — per topline, Jan 16, 2026). The global trial randomized 483 patients ineligible for high-dose chemotherapy and autologous stem cell transplant, 73% of whom had received ≥2 prior lines. (Full presented per-arm medians, CR rates, ORR, and DoR not yet captured from indexed ASCO 2026 reporting.)
Mechanism / class¶
Epcoritamab (Epkinly/Tepkinly; DuoBody CD3xCD20) is a subcutaneous bispecific antibody that engages CD3 on T cells and CD20 on malignant B cells to drive T-cell–mediated killing. It is already FDA-approved as monotherapy for R/R DLBCL after ≥2 prior lines (accelerated approval based on EPCORE NHL-1). EPCORE DLBCL-1 is the confirmatory randomized phase 3 intended to convert that accelerated approval and broaden use into earlier R/R lines.
Practice-changing?¶
Pending. As the first positive randomized phase 3 of a bispecific monotherapy beating chemoimmunotherapy on PFS in R/R DLBCL, it strengthens the case for off-the-shelf epcoritamab in transplant-ineligible patients and supports conversion of the accelerated approval. However, the lack of an OS benefit (HR 0.96) tempers the signal and will be central to discussant and regulatory framing; adoption likely awaits the full presented dataset and regulatory action.
