Headline result¶
At a median follow-up of 35.2 months (data cutoff Oct 20, 2025), frontMIND met its primary endpoint: investigator-assessed PFS favored tafasitamab + lenalidomide + R-CHOP over R-CHOP alone, with HR 0.75 (95% CI 0.59–0.96; P=.0194) — a 25% reduction in risk of progression or death. Median PFS was not reached in either arm; the absolute PFS benefit was 8.2% at 2 years (24-month PFS 71.1% vs 62.9%) and 6.6% at 3 years (36-month PFS 67.3% vs 60.7%). The benefit was deeper in the centrally confirmed DLBCL/HGBL subset (n=773; HR 0.68, 95% CI 0.52–0.88). The key secondary endpoint of event-free survival was also met (HR 0.79, 95% CI 0.64–0.97; P=.0260; 24-mo 65.0% vs 56.7%). Interim OS favored the triplet but did not reach significance (HR 0.85, 95% CI 0.63–1.14; P=.2703; 82 deaths [18.5%] vs 97 [21.7%]; 24-mo 84.1% vs 80.5%). CR/ORR were similar between arms. Safety: grade ≥3 TEAEs 86.7% vs 76.1%, most commonly anemia (22.8%), thrombocytopenia (13.1%), and neutropenia (12.4%), with no new safety signals; treatment discontinuation 5.2% vs 5.4%; R-CHOP dose intensity preserved in both arms.
Mechanism / class¶
Tafasitamab (Monjuvi/Minjuvi) is a humanized, Fc-engineered anti-CD19 monoclonal antibody, already FDA-approved with lenalidomide for relapsed/refractory DLBCL not eligible for transplant (L-MIND). frontMIND moves the CD19 antibody + immunomodulatory backbone into the first-line setting on top of standard R-CHOP.
Discussant takeaway¶
Discussant flagged that the magnitude of OS benefit remains immature and that more data are needed on how prior tafasitamab (anti-CD19) exposure interacts with subsequent CD19-directed CAR T-cell therapy at relapse. Trade-press coverage framed the triplet as a potential "new standard" for newly diagnosed high-risk DLBCL/HGBL, the first regimen in years to beat R-CHOP on PFS in this population.
Practice-changing?¶
Pending. First positive frontline phase 3 regimen to add to R-CHOP and improve PFS in high-risk DLBCL/HGBL in this design, and Incyte plans a 2026 sBLA filing for the 1L indication. But median PFS was not reached, OS is not yet significant, grade ≥3 toxicity is higher, and unresolved CAR-T sequencing questions temper immediate standard-of-care adoption; broad uptake likely awaits regulatory action and maturing survival data.
