Abstract: LBA8500 · Session: Lung Cancer—Non-Small Cell Metastatic, Oral Abstract Session, Fri May 29 2026, 1:00–1:12 PM CT, Hall D2 · Presenter: John V. Heymach, MD, PhD (MD Anderson Cancer Center) · Embargo: May 29 2026

At a glance

  • Sponsor: Dizal Pharmaceutical (Shanghai)
  • PI / presenter: John V. Heymach, MD, PhD — Chair, Thoracic/Head and Neck Medical Oncology, MD Anderson Cancer Center
  • NCT ID: NCT05668988
  • Phase: 3 (multinational, randomized, confirmatory; 15 countries across Asia, Europe, North America, South America)
  • Design: Open-label, randomized 1:1 to sunvozertinib 300 mg PO once daily vs investigator-choice platinum-based doublet (carboplatin + pemetrexed ± pemetrexed maintenance)
  • N: 324 (1:1 randomization)
  • Primary endpoint(s): PFS by blinded independent central review (BICR) per RECIST 1.1
  • Comparator: Platinum-doublet chemotherapy (carboplatin/pemetrexed)
  • Indication: Treatment-naïve locally advanced or metastatic non-squamous NSCLC with EGFR exon 20 insertion mutation (exon20ins)
  • Line: First-line

Headline result

First-line sunvozertinib monotherapy significantly prolonged BICR-assessed PFS vs platinum-doublet chemotherapy, meeting the primary endpoint: median PFS 10.3 months (95% CI, 8.3–14.0) vs 7.5 months (95% CI, 6.7–8.5), HR 0.65 (95% CI, 0.50–0.85; P = .0008) — a 35% reduction in risk of progression or death. Confirmed ORR by BICR was 58.9% (95% CI, 50.9–66.5) with sunvozertinib vs 31.1% (95% CI, 24.0–38.8) with chemotherapy. Adverse reactions were described as generally manageable and reversible, with no new safety signals. Results were published simultaneously in the New England Journal of Medicine.

Mechanism / class

Sunvozertinib (DZD9008, ZEGFROVY) is an oral, irreversible, EGFR exon 20 insertion–selective tyrosine kinase inhibitor that also covers other uncommon EGFR mutations while sparing wild-type EGFR. It received accelerated/conditional approval (China NMPA 2023; US FDA accelerated approval July 2025) for EGFR exon20ins NSCLC after platinum-based chemotherapy; WU-KONG28 is the confirmatory first-line registrational study.

Discussant takeaway

Presenter framing (Heymach): the trial "marks a clinical turning point … offering a practice-changing alternative that may spare treatment-naïve patients from initial cytotoxic treatment." Independent discussant commentary not separately captured in available reporting.

Practice-changing?

Likely yes for 1L EGFR exon20ins NSCLC. This is the first positive randomized phase 3 of a targeted oral agent vs chemotherapy in the front-line exon20ins setting, with a statistically significant PFS benefit, roughly doubled ORR, and simultaneous NEJM publication — positioning sunvozertinib to move from post-chemo (current approved setting) into first line. OS data remain immature.
