Abstract: LBA1 · Session: Plenary, Sun May 31, 2026, Hall B1 · Presenter: Mary-Ellen Taplin, MD, FASCO (Dana-Farber Cancer Institute) · Embargo: lifted 7:00 AM CT May 31, 2026 · Simultaneous publication: NEJM

At a glance

  • Sponsor: Johnson & Johnson (Janssen / Aragon)
  • PI / presenter: Mary-Ellen Taplin, MD, FASCO — Dana-Farber Cancer Institute
  • NCT ID: NCT03767244
  • Phase: 3
  • Design: Randomized, double-blind, placebo-controlled. Perioperative apalutamide + ADT vs placebo + ADT — 6 months neoadjuvant before radical prostatectomy + pelvic lymph-node dissection, then 6 months adjuvant.
  • N: 2,109
  • Primary endpoint(s): Metastasis-free survival (MFS) and pathologic complete response / minimal residual disease (pCR/MRD) — both met
  • Comparator: Placebo + ADT
  • Indication: Newly diagnosed high-risk localized or locally advanced prostate cancer, candidates for radical prostatectomy
  • Line: Perioperative (neoadjuvant + adjuvant)

Headline result

Co-primary endpoints both positive. pCR/MRD by BICR: 8.9% (94/1057) vs 1.0% (10/1052) — patients on the intensified regimen were roughly nine times more likely to have little to no residual cancer at surgery. MFS by BICR: HR 0.80 (95% CI 0.67–0.96; P=0.02) — a 20% reduction in the risk of metastasis or death; investigator-assessed MFS was consistent and stronger (HR 0.74; 95% CI 0.62–0.87; P=0.0004). Subsequent therapy was delayed by ~33 months. Results were presented at the Plenary Session (LBA1) and simultaneously published in the New England Journal of Medicine — widely framed as poised to shift a decades-long paradigm in localized high-risk disease.

Mechanism / class

Apalutamide (Erleada) is an oral, next-generation androgen-receptor (AR) pathway inhibitor (non-steroidal antiandrogen). Already approved in non-metastatic castration-resistant prostate cancer (nmCRPC, SPARTAN) and metastatic castration-sensitive prostate cancer (mCSPC, TITAN). PROTEUS extends AR-pathway intensification from the metastatic/advanced settings into the perioperative (curative-intent) localized setting around radical prostatectomy for the first time.

Discussant takeaway

Discussant Declan Murphy, MB BCh (Peter MacCallum Cancer Centre, Melbourne) framed the data as a potential shift in the standard of care for high-risk localized disease, where systemic intensification around surgery has historically failed to show benefit. Discussion centered on the modest MFS HR magnitude (0.80 BICR) versus the striking pathologic-response separation, and whether MFS will translate to OS with longer follow-up — the recurring "MFS-vs-OS" framing for localized prostate intensification.

Practice-changing?

Likely yes — the first positive phase 3 MFS readout for perioperative AR-pathway intensification around radical prostatectomy in high-risk localized/locally advanced prostate cancer, presented as a Plenary LBA with simultaneous NEJM publication. Positions perioperative apalutamide + ADT as a candidate new standard of care, though regulatory positioning, guideline uptake, and adjuvant-duration questions remain.
