BREAKWATER cohort 3 — Encorafenib + cetuximab + FOLFIRI in 1L BRAF-V600E mCRC

Headline: Encorafenib + cetuximab + FOLFIRI (EC + FOLFIRI) delivered superior response rates and an OS curve trending ~30 vs 15 months over FOLFIRI + bevacizumab in 1L BRAF-V600E mCRC. Establishes the regimen as the new 1L standard for this biomarker-defined subset and pushes the all-comer chemo era out of first-line BRAF-V600E.

At a glance

  • Sponsor / PI / institution: Pfizer (Seagen legacy program); presented by Scott Kopetz, MD Anderson
  • NCT ID: NCT04607421
  • Phase / arms / N: Phase 3, randomized open-label; EC + FOLFIRI vs FOLFIRI + bevacizumab (control); cohort 3 N ~360
  • Primary endpoint(s): OS, PFS, ORR (BICR)
  • Indication / line: Previously untreated BRAF-V600E-mutant metastatic colorectal cancer
  • Comparator: FOLFIRI + bevacizumab (investigator choice mFOLFOX6 + bev permitted)
  • Session: Day 1 (Thu Jan 8, 2026), CRC General Session, Moscone West

Headline result

The January 2026 cohort 3 OS update showed median OS trending approximately 30.3 vs 15.1 months in favor of EC + FOLFIRI (HR ~0.55), with the OS curve still maturing. ORR was substantially improved over FOLFIRI + bev, and PFS hit the protocol-specified hierarchical bar. The differentiator vs FOLFOXIRI + bev is the neuropathy-free profile, given oxaliplatin avoidance.

Mechanism / class

Encorafenib is a selective BRAF kinase inhibitor; cetuximab is an anti-EGFR mAb that blocks the EGFR feedback loop characteristic of BRAF-V600E CRC. The doublet was previously approved 2L+ post-BEACON CRC; BREAKWATER moves the regimen with chemo backbone to 1L.

Trial design deep dive

Study architecture. Phase 3 randomized open-label 1:1; EC + chemo (cohort 2 = mFOLFOX6 supporting 2024 accelerated approval; cohort 3 = FOLFIRI) vs investigator-choice chemo + bevacizumab (mFOLFOX6 + bev or FOLFIRI + bev). Stratification by ECOG (0 vs 1), region, and prior adjuvant therapy. Biomarker-selected: BRAF-V600E mutation by tissue PCR/NGS required.

Endpoint definitions. Primary endpoints PFS by BICR and OS; ORR by BICR served as the basis for cohort 2 accelerated approval. OS analyzed ITT, no censoring for subsequent BRAF-targeted therapy (relevant because control-arm patients could cross over to EC-doublet at progression in clinical practice). Cohort 3 OS at January 2026 readout was interim with ~60% of events — TBD on final maturity.

Key subgroup analyses.

  • Right- vs left-sided primary: consistent benefit per discussant framing; right-sided tumors (overrepresented in BRAF-V600E) still respond.
  • MSI-H vs MSS subset: BRAF-V600E enriches for MSI-H (~30% overlap); benefit consistent in MSS subset where IO is not a competing option.
  • Liver-only vs extensive metastatic disease: TBD on specific HRs.
  • Age (<65 vs ≥65): neuropathy-free profile of FOLFIRI backbone particularly relevant in elderly subset.

Comparator context. FOLFOXIRI + bev (TRIBE-2) was the prior aggressive 1L choice for fit BRAF-V600E patients but carried significant neuropathy/myelotoxicity. EC + FOLFIRI now displaces both FOLFOXIRI + bev (toxicity) and FOLFIRI + bev (efficacy) for biomarker-defined BRAF-V600E.

FDA / regulatory implications. EC + mFOLFOX6 received accelerated approval December 2024 in 1L BRAF-V600E mCRC. Cohort 3 OS data converts this toward full approval and supports a FOLFIRI-based label expansion. NCCN Category 1 expected through 2026.

Discussant takeaway

Howard Hochster (Rutgers) framed the data as the permanent exit of 1L BRAF-V600E mCRC from the all-comer chemo era — biomarker-targeted triplet now reference frame, FOLFOXIRI + bev relegated to non-BRAF-V600E.

Practice-changing verdict

Yes. NCCN guideline update expected through 2026; FDA already approved EC + mFOLFOX6 in 1L BRAF-V600E mCRC (Dec 2024 accelerated approval based on BREAKWATER ORR); cohort 3 OS data converts that into full approval support.
